The OSHP PGY1 Resident Award

  
  Nomination Form

Purpose

To recognize a pharmacy resident who has demonstrated exceptional skills in the area of residency practice.

Criteria

The nominee must be a current PGY1 resident. Residents enrolled in a dual PGY1/PGY2 program, can be considered for this award during their PGY1 year.

Nomination

May be submitted by any member of OSHP, OSHP Board of Directors, or Local Affiliated Chapters. A letter of recommendation from a pharmacy residency director or a program preceptor must accompany the nomination.

Selection

Made by OSHP Division of Professional Affairs; final approval by the OSHP Board of Directors.

Deadline

June 6, 2025

Presentation

OSHP Annual Meeting.

Award

Plaque; Complimentary OSHP Annual Meeting Registration; Complimentary one-year OSHP membership.
